Annual Hutchinson Arts & Crafts Festival – Exhibitor Application

All artists/exhibitors will be asked to sign the ST19 Form (Operator Certificate of Compliance) at Check-In. If you do not currently have a Minnesota State Sales Tax ID #, please enter "In Process" if you are in the process of acquiring one. Please enter "Tax Exempt" if you are selling items that are tax exempt. If you do not feel comfortable submitting your Minnesota State Sales Tax ID # online, please enter "Please Contact" and we will confirm it with you at a later date. To register for a Minnesota State Sales Tax ID #, call the Minnesota Department of Revenue at 651-282-5225.

License Plate of vehicle you will be driving to the show. This helps us identify exhibitor vehicles from attendee vehicles.
